3. Complete the Screener: Before we schedule your clinical evaluation, please submit your basic administrative details below. Our office will use this information to manually set up your secure patient profile and invite you to our secure, HIPAA-compliant clinical portal to complete our brief eligibility screener. This initial request form takes less than a minute to complete.

Important Clinical & Legal Information

Focused Evaluation: This is a focused adult autism diagnostic evaluation. Its purpose is to determine whether the available developmental, behavioral, clinical, and collateral evidence supports an Autism Spectrum Disorder diagnosis. It is not a comprehensive neuropsychological, psychiatric, forensic, disability, or cognitive evaluation.

Telehealth Boundaries: By law, telehealth services are delivered in the state where you are physically located at the time of your appointment. We are authorized to serve clients located in Illinois and active participating PSYPACT states. We cannot conduct evaluations if you are physically located outside these authorized jurisdictions during your sessions.

Diagnostic Neutrality Policy:Your fee purchases a thorough professional evaluation and independent clinical opinion; it does not purchase or guarantee an autism diagnosis. The goal of our practice is clinical clarity, not confirmation.
